What is the Compass Navigation Program?

Healthcare decisions aren’t always simple—especially if you’re coordinating care across locations, don’t have a local primary care provider or want a more personalized level of support. Compass is a tailored healthcare navigation service for individuals and families who want a more seamless, supported experience within Aspen Valley Health’s Network of Care.

Compass provides a central point of contact—helping you navigate services, coordinate care and access appropriate resources efficiently and thoughtfully.

Membership is available to anyone seeking a higher level of guidance and responsiveness in their healthcare experience.

Compass Provides Peace of Mind

Members receive:

  • Direct access to a dedicated nurse practitioner who knows you and your needs
  • Personalized navigation for referrals, scheduling and pre-authorizations
  • Timely clinical access, including visits on short notice when appropriate
  • Care coordination with outside providers, when desired
  • Medical guidance for questions, prescriptions, lab work and imaging
  • Assistance navigating medical records and billing as needed

Support is available in person, by phone or text, via email and through the MyChart patient portal

Program Details

When a question or concern arises, Compass members contact their dedicated nurse practitioner directly.

Your provider will:

  • Listen and triage your needs
  • Provide clinical insight and guidance
  • Prescribe labs, imaging, treatment and referrals
  • Help determine the most appropriate next steps

Compass is designed to simplify healthcare interactions, reduce uncertainty and provide reassurance—especially during moments when timing and clarity matter.

The Compass Navigation Program is ideal for individuals and families who:

  • Spend time in the Roaring Fork Valley seasonally or part-time and may be less familiar with AVH’s Network of Care
  • Do not have an established local primary care provider
  • Coordinate care across multiple healthcare systems
  • Value personalized service and direct access
  • Prefer a more guided, relationship-based healthcare experience
  • Annual household membership: $3,000
  • Includes the enrolling individual, spouse/partner and dependents under age 26
  • Membership covers navigation services only
  • Medical services are billed separately and may be covered by insurance. Members remain responsible for applicable copays, deductibles and coinsurance.
