Do you need financial assistance?

Aspen Valley Health is here to advocate for you if you are having difficulty paying your medical bills. Applications for charity care through the Colorado Hospital Discounted Care Program (HDCP) are available to those who qualify. Aspen Valley Health will assist patients and responsible parties in the completion and submission of any application for medical assistance or charity care determination. Please note: The patient or guarantor is responsible for providing timely, accurate information to comply with HDCP requirements.

Who qualifies?

• Colorado residents
• People who meet income and resource guidelines
• People who are not eligible for Health First Colorado (Colorado’s Medicaid Program) or Child Health Plan Plus (CHP+)
• People who are covered by Medicare or who have other health insurance may be eligible for HDCP.

How to Apply

Applications are accepted and processed only after all other medical assistance programs are exhausted (insurance, Medicare, Medicaid, and other federal, state or local programs). Falsification of information or failure to provide necessary information will result in denial of eligibility and the patient/guarantor will be held responsible for full payment.

Uniform Application

Patients should contact one of our Patient Financial Counselors to complete the application. For assistance, please call the correct counseling line according to your last name (see below), or email eligibility@aspenvalleyhealth.org. Staff are available Monday – Friday 8am to 4pm. Mountain Time.

• For patients with a last name starting with A-L, please call 970.544.1415.

• For patients with a last name starting with M-Z, please call 970.544.1543.

In instances where charity care is not approved, Aspen Valley Health would be happy to discuss a payment plan with you.
