Living with diabetes can be challenging—we are here to help.

A new diabetes diagnosis can be especially overwhelming. The Diabetes Education Program at Aspen Valley Health is designed to help you gain the knowledge and skills you need to feel confident about caring for yourself and staying healthy over a lifetime.

Our Certified Diabetes Care and Education Specialist (CDCES) helps you navigate your way through healthy eating and meal planning, physical activity, medication use and the latest diabetes technologies for the best health outcomes.

The four key times to see a diabetes care and education specialist are:

  • At diagnosis
  • Annually
  • When a new complicating factor arises
  • When a transition in life and care happens

Diabetes treatment and technologies are rapidly evolving. Regular visits ensure your management plan is the best one for you.

Services Offered

We provide individual counseling for people with Type 1 diabetes. These sessions teach intensive management to allow greatest flexibility for a happy and healthy lifestyle.

Topics include:

  • medications that fit your lifestyle
  • new glucose technologies
  • healthy nutrition and physical activity
  • increasing your time in target to stay healthy over a lifetime

Offered in an individual setting, this program teaches you how to take care of diabetes day-to-day. It is designed for people with Type 2 diabetes who need to update skills or begin to manage a new diagnosis of diabetes. This program focuses on ways to improve quality of life and prevent complications.

You will learn:

  • what is diabetes and how do you manage it
  • healthy meal planning
  • incorporating physical activity into your life
  • how to monitor your progress and which new technologies are available for you

For expecting mothers who develop diabetes in pregnancy.

You will learn:

  • what is gestational diabetes and how do you manage it
  • healthy meal planning and physical activity
  • monitoring blood glucose
  • using medications if needed
  • preventing Type 2 diabetes

Yes, you can monitor your blood sugar without finger pricks! CGM technology is revolutionizing diabetes care and helping people to really see the areas they need to time in target to stay healthy over a lifetime.

Our CGM Program includes two systems and allows you to try a new technology for 10-14 days and collect data 24/7 that helps you and your practitioner problem solve to make informed decisions about your diabetes management.

How to Register

Your primary care provider can refer you to the Diabetes Education Center or you can call to inquire. All programs are offered in Aspen and Basalt, in both English and Spanish.

We accept Medicare, Medicaid and private insurances.

Meet Our Diabetes Educator

Nutrition Experts

A registered dietitian (RD) is a food and nutrition expert, translating the science of nutrition into practical solutions for healthy living. Registered dietitians have a degree in nutrition from well-respected, accredited colleges and universities. RDs must also complete a supervised examination administered by the Commission on Dietetic Registration and earn professional education credits to maintain registration. All visits with a registered dietitian require an appointment and physician referral.
